Medical Spa New Patient Forms

Here are our forms for all new patients seeking our wellness services. The required forms will vary based on the services you are seeking. These services include wellness consultations, IV therapy, vitamin shots, microneedling, peptide therapy, PRP therapy, NuCalm therapy, Botox/Dysport injections, and dermal fillers. We ask that all new patients bring a signed financial policy, as well as applicable history and consent form(s) to your appointment. If you’d rather complete the paperwork at our clinic, we request that you arrive 15 minutes early to your schedule appointment. Trua Health & Wellness offers Bioidentical Hormone Replacement Therapy, IV nutritional therapy, peptide therapy, NuCalm, as well as medical spa services like Botox/Xeomin injections, facials, PRP therapy, Plasma Pen, Skin Pen Microneedling, and vitamin shots. We are focused on helping our patients optimize their health and continue to make them feel great both inside and out.

We recommend IV therapy because it provide hydration with any vitamins and minerals you may be getting. Both IV therapy and vitamin shots are also recommended over oral supplements because nutrients administered intravenously or intramuscularly can bypass the gut so you absorb everything into your bloodstream.

Trua Health & Wellness offers free 15 minute phone consultations with our provider Julie Kane. During this scheduled phone appointment, Julie is happy to discuss with you why you can considering treatment and what the best options might be. If you don’t think 15 minutes is enough time to ask all your questions and get the information you need, we highly recommend scheduling an office consultation with Julie to sit down and discuss your wellness goals at length. We can also run any necessary labs at an office consultation, to get Julie as much of your medical information as possible to make the best recommendations. Microneedling recovery varies with every person based on how healthy their skin is. It typically takes 1 day for the redness to go away and 2-3 days for the peeling to minimize. Your skin might feel tight and dry when the numbing agent wears off after treatment, so we recommend using the provided skin care product to help with faster recovery. It is okay to use your own moisturizers 24 hours after treatment, but avoid using any products that contain retinol. There is no downtime required for this treatment, so you can have the treatment and maintain the same activity level. Excessive sweating, however, is discouraged for the first 2-3 days so your skin can heal properly.

We recommend you keep your head upright for 4-6 hours after injections. Do not massage or put pressure over areas that were injected, like hats or headbands. Avoid excessive heat or cold for 24 hours, and avoid vigorous exercise for 24 as well. Do not receive facials, waxing, or laser treatments for 2 weeks post-treatment. Results can be seen anywhere from 3 days up to 2 full weeks for Botox. Do not attempt to “fix” a treatment until 14 days have passed.

Unfortunately, everyone has different needs when it comes to nutritional supplementation. We do list the intended functions of all the supplements we have available for purchase in our item descriptions as well. This is convenient if you would like a quick overview of how each of our supplements work!

We accept all insurances for our services and some of our IV therapy treatments. We are in-network with Medicare, Medicaid, Aetna, Premera Blue Cross Blue Shield, Cigna, United Healthcare, Tricare West Region, and the VA. Talking with your insurance company is a great idea if you aren’t sure whether a visit would be fully covered. All our wellness therapies are paid for out-of-pocket, so follow our Instagram and Facebook pages for promotional deals!
